The submission of this form does not guarantee your reservation of your preferred week.  You will be contacted by the Kentucky FFA Leadership Training Center for confirmation.  If you are not contacted, please contact the LTC to confirm receipt of this form.

A complete reservation form must be submitted to the Kentucky FFA Leadership Training Center with payment prior to May 15th.  The reservation packets will be mailed to you by April 1, 2013.

If deposit is not received by May 15 your spots will be given to to other chapters.
